치매환자의 인지기능, 일상생활 수행능력 및 문제행동 분석

Abstract

This study is aimed at analysis cognitive function, activities of daily living and behavioral problem in dementia patients in dementia center to offer better understanding to community about dementia center and encourage an affirmative concept in using dementia center and nursing home to relieve the pain of protector and provide a better environment to dementia patient. The data for this study were collected through a research which was conducted to 72 dementia patients in D dementia center in Gwang-ju, from January to December, 2005. To gather personal records for dementia patients of the first and second half for year, research were done with personal interview used as basic form for medicare service. The instrument used for this study, MMSE-K (Koreanized mini mental state examination) for the aged developed by Park Jong-Han and Kwon Young-Chul(1989) who translated and revise Mini Mental State Examination (MMSE) by Folstein(1975), Barthel Activities of Daily (BADL) by Barthel (Mahoney and Barthel, 1965) and NPI-Q which is easy way of NPI by Cummings(1989) were used for checking stress of protector and behavioral problem in dementia patient. And we also have GDS (Global Deterioration Scale) by Reisberg group(1982) for overview of degrade. The data collected were analysed using SPSS Win to analyse data and Sigma plot to show figures. General characteristics of subjects, cognitive function, activities of daily living, behavioral problem with dementia patients in D dementia center and stress of protector were analysed by descriptive statistics. The correlations between cognitive function and activities of daily living, behavioral problem of subject and stress of protector were analysed by Pearson's correlation The result of study was as follows : 1. General characteristics of subjects, female was 90.3% and mean-age was 79.2, mean-age of male's was 73.2 and female's was 79.9. We have a most large group in the eighties as 44.4% and then comes seventies, sixties and nineties in that order. Education standards for subjects show that 79% were uneducated. 2. The score in MMSE-K was 3～28, mean score was 16.5. And the score in BADL was 0～19, mean score was 5.2. The score in NPI-Q(behavioral problem of subject) was 1～17, mean score was 7.2. The score in NPI-Q(stress of protector) was 1～25, mean score was 9.1. Night act ranked high in pattern of behavioral problem. and then comes delusion and anxiousness/aggression in that order. The score in GDS was 3～7, mean score was 6.1 which is pointing out that almost every subject is suffering medium-high grade dementia. 3. The relationship between cognitive function and activities of daily living of subject, activities of daily living of subject and global deterioration scale, cognitive function of subject and global deterioration scale, behavioral problem of subject and stress of protector was in significance level statistically. This study showed that cognitive function of subject, global deterioration scale and stress of protector was in significance level statistically. So, to get rid of the exhaustion with a long-drawn-out nursing, family discord and the grim realities of life, I expected this study to be used as a preliminary data to offer better understanding to community about dementia center and encourage an affirmative concept in using dementia center and nursing home.
