소아에서 보완대체의학의 이용실태와 보호자의 인식

Abstract

목적: 최근 대체의학과 보조치료의 이용이 증가되고 있는 추세로 소아에서의 이용률과 이용양상, 보호자들의 인식을 조사하여 올바른 사용을 유도하고자 하였다. 대상 및 방법: 2003년 7월 한달간 광주소재 아파트 주변의 1개 초등학교 794명의 학부모를 대상으로 25문항의 설문을 조사하였으며 병원 치료외의 한방치료를 포함한 모든 보조치료를 대체의학으로 전제하였다. 결과: 총 794명중 대체의학에 대해 긍정적인 경우 60.3%, 사용해 본 경우 35%이었고 종류로는 한약제 21.5%, 보조식품 11%, 비타민 10.6%, 선식 3.9%, 침술 3.9%, 체를 냄 3.0%, 수지침 1.6% 순이었다. 질환별은 영양불량 33.3%, 아토피 피부염 31.3%, 관절 통증 31.3%, 알레르기 비염 28.8%, 비만 26.3%, 천식 24.1%, 야뇨증 23.1%, 변비 16% 순이었고 페렴, 빈뇨, 간질에는 사용자가 없었다. 사용자 중 아이의 건강에 대한 생각별로는 건강함 77.3%, 보통 21.2%, 건강하지 못함 1.4%이었고 평소 건강에 대한 관심은 매우 관심 있음 27.7%, 관심 있음 56.5%, 보통 15.8%이었다. 사용자중 아버지 학력은 대학이상 71.6%, 고졸이하 28.4%이고 어머니의 학력은 대학이상 48.2%, 고졸이하 51.8%이었다. 사용자중 아버지 종교는 무교 47.8%, 기독교 24.6%, 불교 20.3%, 카톨릭 5.8%순이었고 어머니의 종교는 무교 33.6%, 기독교 32.5%, 불교 22.6%, 카톨릭 11.3%순이었다. 사용자중 경제수준의 경우 100만원미만 7.2%, 100～200만원미만 28.8%, 200～300만원미만 38.1%, 300만원이상 25.9%이었다. 사용이유로는 질병의 예방 목적 33.5%, 병원치료에 만족하지 못하여 치료를 바꿀 때 21.2%, 병원치료의 보조제 20.5% 순이었고 이용 경로는 주변사람의 권고 64.7%, 방송매체 20.9%이었다. 사용 후 만족도는 만족 30.2%, 보통 54.3%, 불만 15.5% 이었고 만족 이유로는 효과가 있어서 70.2%로 가장 높았고 부작용이 적어서, 비용이 저렴하여 순이었다. 재사용 의향의 경우 있는 사람 67.6%, 없는 사람 20.5% 이었다. 대체의학과 병원치료와의 관계에 대한 생각은 병원치료와 병행시 좋음 63.5%, 병원치료에 우선 중점을 두어야 함 22.9%, 병원치료와 병행시 해로울 것은 없음 10% 순이었다. 결론: 보호자의 학력, 경제 수준, 종교와 연관성을 보이지 않고 주로 치료의 보조와 질병의 예방으로 이용되고 있는 대체 의학에 대한 부작용, 효과, 안정성에 대한 확립이 필요하며 이에 대한 의료진의 관심이 요구된다.|Background : Recently, the use of alternative medicine is widespread among children. The interest in alternative medicine is growing. In the USA and Canada, studies showed that 34% of adults and 11% of children use alternative medicine. This objective was to investigate the distribution and patterns of alternative medicine use by children alone with the parental attitude towards alternative medicine Method : In July, 2004, parent of 794 sixth grades attending elementary school. Parents of the children completed a questionnaire, The questionnaire was consisted of the prevalence and types of alternative medicine used in the children, the motivation of using alternative medicine, the costs and whether parents were satisfied with alternative medicine. Results : Among the respondents, 478 parents(60.3%) appreciated alternative medicine. 278 of them(35%) have received alternative medicine at some point in their lives. The diseases and problems, for which parents used alternative medicine for their children, were nutritional deficiency, atopic dermatitis, athralgia, allergic rhinitis, obesity and asthma. Significant differences between alternative users and non users have created the prejudice against group of people practicing alternative medicine(e.g. well educated, high incomes)(P < 0.05). Motivation for choosing alternative medicine were prevention of diseases(93/278), dissatisfaction of conventional medicine(59/278), and complementary therapy to hospitalization(57/278). The course of use was recommendation of friends(180/278)(P < 0.05). The cause of satisfaction was effect of alternative medicine(P < 0.05). Conclusion : A substantial number of parents have used complementary/altenative medicine, or would like to do so. This suggests that complementary/alternative medicine is at great interest. Further studies are needed to prove or disprove the efficacy of complementary/alternative medicine.
