코로나-19 바이러스 감염증(COVID-19) 치료병원 간호사의 외상 후 스트레스와 관련요인

Abstract

Objective: Coronavirus infection-19 is a worldwide epidemic and an ongoing infectious disease to date. Previous studies have been addressed on the psychologic burden such as stress, burnout of nurses during the coronavirus infection-19 outbreak period. However, the data on post-traumatic stress and related factors were limited in Korea. The purpose of this study is to investigate the post-traumatic stress (PTSD) of hospital nurses during the outbreak of coronavirus infection-19 and to investigate related factors. Methods: The subjects of this study were 300 nurses who worked in national treatment beds or corona specialty hospitals during the corona-19 outbreak in 3 general hospitals. Self-reported questionnaire were used to collect data on PTSD, general characteristics and work-related information. The collected data were analyzed using the SPSS version 26 statistical program. The statistical analysis methods were frequency analyses, chi-square tests, and multiple logistic regression analyses. Results: The overall PTSD scores was 20.68±19.5 and 36.7% of participants were the high-risk group. In multiple logistic regression analyses, the odds ratio (OR) for PTSD in nurses working in the national treatment bed was significantly higher than that those worked in other departments(OR: 16.31, 95% CI: 3.79-70.32). The OR for PTSD in nurses who answered that they had few nursing staff was significantly higher than those who answered that they had moderate or large number(OR : 3.03, 95% CI : 1.01-9.10). The OR for PTSD in nurses who answered that they had suspicious symptoms in the presence or absence of suspected symptoms of COVID-19 was higher than that those without symptoms(OR = 3.83, 95% CI: 1.89-7.75). Conclusion: The factors related to the PTSD were the work department, the nursing personnel, and the presence or absence of suspected COVID-19 symptoms. These results could be used to manage PTSD and provide psychological support of nurses during infectious disease epdidemic such as COVID-19. | 본 연구는 코로나-19 발생기간동안 병원 간호사의 외상 후 스트레스를 파악하고, 관련요인을 파악하고자 하였다. 본 연구의 대상자는 코로나-19 발생기간동안 국가치료병상을 운영하거나 코로나 전담병원을 운영했던 병원 3곳에 근무하는 간호사 300명이었으며, 자료수집기간은 2020년 9월 3일부터 9월 16일까지 였고, 외상 후 스트레스를 포함한 구조화된 설문지를 배포하고 직접 기입하도록 하였다. 통계자료는 SPSS version 26 통계 프로그램을 이용하여 분석하였다. 분석방법은 빈도분석, 카이제곱 검정, 이분형 로지스틱 회귀분석을 이용하였다. 통계적 유의수준은 p<0.05을 기준으로 검정하였다. 본 연구의 결과 대상자의 외상 후 스트레스는 점수는 88점 만점에 평균 20.68점, 표준편차 19.5점 이었다. 25점 이상의 고위험군은 110명(36.7%), 25점 미만의 정상군은 190명(63.3%) 이었다. 코로나-19기간동안 병원간호사의 외상 후 스트레스와 관련된 요인은 근무부서, 간호 인력, 코로나-19 의심증상 유무였다. 대상자의 근무부서는 국가치료병상의 교차비(OR)가 일반병동보다 유의하게 높았다(OR : 2.85, 95% CI : 1.21-6.71). 간호인력이 적다고 답변한 대상자의 교차비(OR)가 보통이거나 많다고 대답한 대상자보다 유의하게 높았다(OR : 3.03, 95% CI : 1.01-9.10). 코로나-19 의심증상 유무에서, 의심증상이 있었다고 답변한 대상자의 교차비(OR)가 증상이 없었다고 답변한 대상자보다 유의하게 높았다(OR : 3.83, 95% CI : 1.89-7.75). 본 연구의 결과에 따르면 근무부서가 타 부서보다 국가치료병상에 근무할 때, 간호인력이 적었을 때, 코로나-19 의심증상이 있었을 때 외상 후 스트레스 점수가 높았다. 이 결과에 근거하여 전국 각 병원의 근무부서 중에서 국가치료 병상에 근무한 간호사의 외상 후 스트레스에 대한 지속적인 대책이 필요하며, 이는 조직 차원에서 심리지원 프로그램, 보상, 처우개선 등 여러 가지 외상 후 스트레스를 줄일 수 있는 방안을 마련해야 할 것이다. 또한 국가차원에서 간호 인력 충원 방안을 마련하여 간호 인력을 충분하게 보장하여야 할 것이다. 코로나-19 관련 의심증상이 나타났을 경우 병원에서 무료검진을 시행하도록 하여 본인이 코로나-19에 걸렸을 수도 있을 것이라는 불안감을 줄여주고, 더불어 코로나-19의 병원 내 감염도 효과적으로 줄일 수 있을 것으로 예상된다.
