전인적 간호 돌봄의 현상학적 분석과 간호 인력관리에의 시사점

Abstract

The purpose of this study is to provide implications for nurse manpower management through phenomenological analysis of nurse 's holistic care. Recently, nurses have changed their values ​​about nursing care capacity based on changes in new medical technologies such as U-Health, nurse's caring value and nightingale mentality. As a result of this, There is a growing need. The purpose of this study was to apply focus group interviews to nurses at c university hospital in Gwangju using qualitative analysis as a qualitative research method for nursing care. As a result, it was classified as 172 essential meaning units through the study on the concept of caring experience and care, and 15 topics were derived through the process of reclassification and integration. This categorized them as a collection of three central themes such as' the concept of care that hospital nurse thinks', 'conflict between normative theory and actual field', and 'humanistic care for patient' s healing '. The implications of this study are as follows. First, it is necessary to develop and apply an intervention program to establish nurses' care philosophy. Second, it is necessary to provide education for improving working environment and strengthening capacity so that nurses' caring act can be performed smoothly. Third, it is necessary to create a nursing organization culture in which caring activities can be freely expressed and accepted mutually. Fourth, nurses' turnover is a big problem, and it is difficult to operate. Therefore, it is necessary to develop a program that can evaluate the value of care when hiring a new nurse.|본 연구는 간호사의 전인적 돌봄에 대한 현상학적 분석을 통하여 간호사의 인력관리에 대한 시사점을 제공하는데 목적을 두고 있다. 최근 간호사들은 U-Health등 새로운 의료기술의 변화와 간호사의 돌봄가치 및 나이팅게일 정신 등에 기초한 간호돌봄 역량에 대한 가치관이 변화하면서 이직 및 이직의향이 높아지고 있으며, 또한 고령사회가 진전되면서 이러한 간호의 돌봄에 대한 필요성은 높아지고 있다. 이에 본 연구는 간호의 돌봄에 대한 질적연구 방법으로서 현상학적 분석을 적용하여 광주 소재 c대학병원의 간호사를 대상으로 포커스그룹 인터뷰를 시행하였다. 그 결과 돌봄 경험과 돌봄의 개념에 대한 연구들을 통해 172개의 본질적 의미단위로서 분류되었고 이후에 재분류 및 통합하는 과정을 거쳐서 15개의 주제를 도출하였다. 이는 다시 ‘병원간호사들이 생각하는 돌봄의 개념’, ‘규범적 이론과 실제 현장에서의 갈등’, ‘환자의 치유를 위한 인본주의적 돌봄’ 등의 3개 중심의미의 주제모음으로 범주화하였다. 이에 본 연구의 시사점은 다음과 같다. 첫째, 간호사의 돌봄 철학의 정립을 위한 중재 프로그램의 개발 및 적용이 필요하다. 둘째, 간호사들의 돌봄 행위가 원활하게 수행될 수 있도록 근무환경을 개선하고 역량강화를 위한 교육 제공이 필요하다. 셋째, 돌봄 행위가 상호간 자유롭게 표현되고 수용할 수 있는 간호조직 문화를 조성하는 것이 필요하겠다. 넷째, 간호사들의 이직이 큰 문제점으로 되어 운영상 어려움이 크다고 본다. 따라서 신규간호사 채용 시 돌봄에 대한 가치관의 평가가 가능한 프로그램 개발이 필요하다.
