건강신념모형을 적용한 치과위생사의 감염관리 수행도 관련요인

Abstract

본 연구는 치과의원에 근무하는 치과위생사를 대상으로 감염관리 현황 및 수행도 관련요인을 알아보기 위해 수행되었다. 조사대상은 광주광역시 488개소 치과의원 중 구별비례층화추출법으로 160개소에 근무하는 치과위생사 278명을 대상으로 자가 보고형 설문조사를 통해 자료를 수집하였다. 일반적 특성과 감염관련특성, 건강신념모형의 구성요소를 포함해 감염관리 수행 관련요인을 조사하였다. 일반적 특성과 감염관련특성의 각 문항은 빈도분석, 감염에 관한 수행도와 중요도 및 감염관리 건강신념은 평균과 표준편차를 분석하였다. 일반적 특성에 따른 수행도와의 비교검정은 t-검정, 분산분석, 감염관리 중요도와 건강신념과 수행도와 관계성은 피어슨 상관관계 분석을 하였다. 최종적으로 감염관리 수행에 영향을 미치는 요인을 알아보기 위해 위계적 다중회귀분석을 이용해 네가지 모델을 제시하여 분석하였다. 대상자 대부분의 연령층은 26-30세로, 근무기관의 치과의사 수 1인에 근무하고 있는 총 경력 1-5년차가 53.6%(149명)이었다. 근무처 총 치과위생사 수는 3-4명으로 64.0%(178명)이었다. 감염관리 수행에 영향을 주는 요인으로 치과위생사의 연령이 높을수록, 감염질환에 관한 문진을 항상 할수록(B=.21, p=.015), 감염관리에 있어 손씻기(B=.14, p=.026)와 개인보호장구 착용(B=.17, p=.043)에 관한 중요도가 높을수록 수행도가 높았다. 건강신념으로는 감염예방에 관한 행동의도(B=.13, P=.002)가 높을수록 수행도가 높았으나, 인지된 장애성(B=-.16, P<.001)이 높을수록 감염관리 관한 수행도는 낮았다. 결론적으로 의원급 치과위생사의 감염관리 수행도로 ‘보통’ 이상이었다. 본 연구 결과를 고려해 볼 때 치과위생사의 감염관리에 대한 중요성을 증가시키거나, 행동의도를 고취 시키고, 관련된 장애요인을 제거한다면 감염관리 수행도가 높아질 것으로 판단된다.|Objectives: The purpose of this study was to investigate the associated factors with the performance of infection control based on the Health Belief Model among the dental hygienists in dental clinics. Material and Methods: Using proportional stratified sampling, 278 dental hygienists who worked in 160 dental clinics in Gwangju were selected. General characteristics, infection related characteristics and performance of infection control were collected by self-reported questionnaires. T-test, ANOVA, correlation analysis and multiple regression analysis were performed to evaluate the related factors with the performance of infection control. Results: The mean performance score was 3.51±0.55. Of all five performance domain, the mean score for use of personal protective equipment was the lowest. In multiple regression models, performance scores were significantly higher in older subjects and subjects who interviewed about infectious disease history when they treat patients. With regard to Health Belief Model, perceived barrier was negatively associated with the performance (β=-.16, p<.001), and importance of infection control about hand hygiene (β=0.14, p=.026), use of personal protective equipment (β=0.17, p=.043) and intention of action were positively associated with the performance (β=0.13, P=.002). Conclusion: These results can be used as evidentiary materials for improving quality of prevention of infection control. The dental hygienists health activities might be improved by enhancing the importance of the infection control and intention of action about prevention.
