노인의 정신건강과 낙상경험의 관련성

Abstract

Objectives: The purpose of this study was to investigate the relationship between mental health and experience of falling in some local elderly people using the 2013 community health survey data Methods: A total of 13,384 out of 32,212 adults aged 19 and over, excluding those under 65 years of age, were selected for the final study. Mental health, and experience of falls. The data were analyzed using frequency analysis, χ2 test, and bivariate logistic regression analysis. Results: As a result of this study, 21.5% experienced falls. 30.7% of households including kitchens, 32.5% of households including stairs, and 24.2% of agricultural facilities. 40.2% experienced depression, and 35.2% experienced suicidal thoughts. The factors influencing the fall experience were the depressive mood, experience of suicidal ideation, basic monthly income, monthly household income, economic activity, lack of necessary medical services, subjective health status, and number of chronic diseases. The odds ratio for experiencing depression was 1.51 (95% CI: 1.19-1.91), and the odds ratio for experiencing suicide was 1.34 (95% CI: 1.09-1.65). Conclusion: The results of this study were as follows. First, in order to prevent falls in the elderly, mental health should be considered in relation to depression and suicide as well as physical approach. |본 연구는 2013년 지역사회건강조사 자료를 이용하여 일부 지역 노인의 정신건강 및 낙상경험과의 관련성을 파악하고자 하였다. 2013년 지역사회건강조사 전라남북도 자료를 통합하여 이용하였으며 만 19세 이상 성인 대상 32,212명 중 만 65세 미만 대상자를 제외한 13,384명을 최종 연구대상자로 선정하였다. 정신건강 및 낙상경험과의 관련성을 알아보기 위해 빈도분석, χ2검정 및 이분형 로지스틱 회귀분석을 이용하여 분석하였다. 본 연구결과 21.5%에서 낙상을 경험하였으며, 낙상장소로는 화장실. 부엌을 포함한 집 안 30.7%, 계단을 포함한 집 밖 32.5%, 농업시설 24.2%로 나타났다. 우울을 경험한 적이 있는 경우 40.2%, 자살생각의 경험이 있는 경우 35.2%에서 낙상을 경험하였다. 낙상 경험에 영향을 주는 요인으로는 정신건강 변수인 우울감 경험, 자살생각 경험과 기초수급자 여부, 월 가구소득, 경제활동 여부, 필요의료서비스 미수진 여부, 주관적 건강상태, 만성질환 수로 파악되었다. 우울감 경험이 있는 군의 낙상경험 교차비는 1.51(95% CI: 1.19-1.91)이었고, 자살생각 경험이 있는군의 낙상경험 교차비는 1.34(95% CI: 1.09-1.65)이었다. 일부지역 노인에서의 정신건강 경험이 낙상경험에 영향을 주는 것으로 나타나 노인에서의 낙상 예방을 위해 신체적 접근뿐만 아니라 우울, 자살 등과 관련된 정신건강 측면의 접근이 병행되어야 하며 정신건강 수준에 따른 낙상관련 예방교육, 관리 및 치료를 통해 낙상으로 인한 손상 위험성을 낮춰야 할 것이다[8,23].
