복강경 하 더글라스와를 통한 부속기 종양 절제술에 대한 고찰

Abstract

배경 : 본 연구는 복강경 하 부속기 종양 수술 후 더글라스와 절개를 통해 절제한 조직을 복강 밖으로 제거하는 수술법의 결과를 확인하기 위함이다. 대상 및 방법 : 2011년 1월부터 2012년 4월까지 조선대학교병원 산부인과에서 동일한 술자에 의해 복강경 하 부속기 종양 수술 후 더글라스와 절개를 통해 절제 조직을 제거한 환자 110명을 대상으로 시행하였다. 결과 : 환자들의 평균 연령은 41.5세였다. 술 전 시행한 초음파 검사 상 종양의 크기는 3cm~27cm까지 다양했으며, 수술 시간은 평균 61.3분, 평균 입원 기간은 6.2일이었다. 110명의 환자 중 82명이 난소-나팔관 절제술 또는 난소 절제술, 19명은 난소 종양 절제술, 2명은 쐐기 절제술, 4명은 종양 적출술을 시행하였다. 조직 검사 결과, 자궁내막종이 23명(20.9%)으로 가장 많았으며, 기형종이 21명(19%), 장액성 낭성종 14명(12.7%), 점액성 낭성종이 10명(9.1%)이었다. 기타 조직 검사 결과는 출혈성 낭포, 황체낭, 부난관낭 등이 있었다. 수술 환자 중 더글라스와 절개에 따른 방광이나 직장 손상 또는 봉합 부위 염증 등의 부작용이 있었던 환자는 한 명도 없었다. 결론 : 복강경 하 부속기 종양 수술 후 더글라스와 절개를 통한 절제 조직의 제거는 쉽고 안전하고 효과적이며 미용적으로도 우수한 방법 중의 하나로 미세 침습적 복강경 수술법으로 선택할 만하다.|Objective: To evaluated the result of laparoscopic surgery of adnexal tumors and removing excised tissue through Douglas pouch incision.

Methods: 110 patient who had visited Chosun University Hospital department of Obstetrics and Gynecology between January, 2011 to April, 2012 were involved in the study. Patient had laparoscopic surgery on the adnexal tumor and excised tissue was removed through Douglas pouch incision by single surgeon.

Results: The mean age of patients were 41.5years. The size of the tumor varied from 3cm to 27cm in size using ultrasonography. The mean operation time was 61.3 minutes and the mean admission period was 6.2days. 82 patient had salpingo-oophorectomy or oophorectomy, 19 had ovarian cystectomy, 2 had wedge resection and 4 had tumor enucleation. Histologic result reveal endometriosis in 23 patients(20.9%). teratoma in 21(19%), serous cystadenoma in 14(12.7%) and mucinous cystadenoma in 10(9.1%). Other histologic finding includes hemorrhagic cyst, luteal cyst and paratubal cyst. There was no complication including bladder or rectal injury, suture site infection.

Conclusion: Laparoscopic surgery of adnexal tumor and removal by Douglas pouch incision is a simple, safe and efficient method that could be considered as an alternative minimally invasive technique.
