Abstract

Background and Purpose: Sleep-disordered breathing (SDB) is suggested to have a strong association with ischemic strokes. Risk factors, stroke subtypes, stroke lesion distribution and the outcome of SDB in stroke patients are not fully studied in Korea. Methods: We prospectively studied 293 patients (159 men, 134 women; mean age, 68.4 ± 10.5 years) with acute ischemic stroke. Cardiovascular risk factors, stroke severity, sleep-related onset of stroke and distribution of stroke lesions were assessed. Stroke severity was assessed by National Institute of Health Stroke Scale (NIHSS) and modified Rankin Score (mRS). The apnea-hypopnea index (AHI) was determined 6.3 ± 2.2 days after stroke onset with a portable sleep apnea monitoring device. Results: The prevalence of SDB (determined by AHI ≥10) was 63.1 % (111 men, 74 women). The SDB group showed older age, higher NIHSS and mRS, higher incidence of bulbar weakness and sleep-related onset of stroke. Stroke outcome which was measured by 3-month mRS was worse in the SDB group. Distribution of the stroke lesion or specific stroke syndrome did not show any correlation. Alcohol consumption and atrial fibrillation correlated significantly with the SDB in multivariate analysis. Conclusions: We suggest that the SDB may be a cause or result of the ischemic stroke.
