혈청 Transferrin saturation(TSAT)과 신기능(GFR)의 관계

- Abstract
- Background : Anemia is common in patients with renal failure. but it is not clear the relationship between serum transferrin saturation(TSAT) and estimated glomerular filtration rate(eGFR). This study was performed to inqure the relation between serum transferrin saturation and estimated glomerular filtration rate.
Methods : A retrosepective analysis was conducted to assess the development of chronic kidney disease(CKD) within the popluation of 48 subjects (25 males, 23 females, in adult) in chosun university outpatient. The data were collected between January 2007 and December 2007. Multivariate logistic analysis were performed to analysis the influence of serum transferrin saturation(TSAT) and estimated glomerular filtration rate(eGFR) after adjusting age, Hemglobin, Hematocrit, Erythropoietin, Body mass index, proteinuria, hematuria. we defined the glomerular filtration rate by the method of Cockcroft and Gault.
Results : The mean level of hematocrit at CKD stage 1 and 2: 36.69±5.18 %, stage 3: 31.76±5.06 %, stage 4: 28.95±4.31 %, stage 5: 25.38±3.41% in this subjects. The mean level of serum ferritin, indirect measurement of stored iron, at CKD stage 2: 343.94±428 ng/mL, stage 3: 190.89±160 ng/mL, stage 4: 283.5±125 ng/mL, stage 5: 285.17±178 ng/mL. The mean level of Transferrin saturation, give a measure of iron available to the bone marrow, at CKD stage 2: 58.55±14.48 %, stage 3: 28.07±9.16 %, stage 4: 28.08±10.78 %, stage 5: 22.17±14.08 %. eGFR correlated postively with Hct(r=0.616, P<0.001) and T.S.A.T(r=0.618, P < 0.001). This finding suggest that the lower hematocrit and the lower T.S.A.T, the greater was the risk of developing ESRD.
Conclusions : Subjects with low hematocrit have a significant increased risk of ESRD and low transferrin saturation increased risk of ESRD
