수술실 간호사에 의한 수술 전 정보제공이 환자의 정서상태와 통증에 미치는 영향

- Abstract
- This study prepares an educational program which helps the patients reduce anxiety on surgery which is considered as one of the biggest emotional crises and increase reliability on surgery by increasing positive effect in recovery process. Then it asks nurses to provide information on nursing before surgery to identify the effect of nursing intervention on emotional status before surgery and pain after surgery, which is designed to contribute to enrichment of whole-person nursing and autonomy in special fields.
The study targets the patients who are going to have surgery under general anesthesia or spinal anesthesia in C university hospital located in Gwangju city from August 29, 2005 to September 30, 2005.
After the survey of the subjects who fit for this study, it targets 42 patients who were provided information for fifteen minutes one day before surgery and 43 ones who were not provided information at all. A total number of the subjects were 85.
This study used various tools to measure anxiety and anxiety factors, depression and vital signs in recovery rooms and operation rooms. To measure pain after surgery, it used VAS one day after surgery.
For analysis of data, it used SPSS program and the results were managed on computers. General characteristics of the subjects were calculated as real number and percentage, and to demonstrate similarity, it used χ2-test. To demonstrate the hypothesis that there is a difference between the two groups, it used t-test.
The results of the study are presented as follows:
For the experimental group who were provided information one day before surgery, there was no significant difference in dispositional anxiety (p=.772), status anxiety(p=.385), anxiety factors (p=.318), and depression(p=.318) in comparison with the control group who were not provided information at all. However, in respect to vital sign as a physiological response of emotional status on the day of surgery, there was a significant difference in systolic blood pressures (p=.020) and diastolic blood pressures(p=.024). And there was no significant difference in vital sign at operation room and pain after surgery, but the experimental group showed lower results than control group. It means that emotional status before surgery is related to pain after surgery and suggests that providing information is effective in emotional status and pain of the patients. So when the results are applied clinically, they will contribute to increase of effectiveness and improvement of quality of nursing intervention.
As a result of the study, the following suggestions are presented :
1) We have to develop diverse studies and organized tools in respect to how to provide information before surgery.
2) To increase the effect of hypothesis demonstration, hospitals have to prepare repetitive research by increasing the number of samples.
3) We need further research for analysis on how to provide information and the periods and methods to use measuring tools.
