合倂症이 同伴되지 않은 急性 腎盂腎炎 患者에서 Ciprofloxacin과 Cefotaxime의 效果 比較

Abstract

Background The optimal antimicrobial regimen and duration of therapy for acute uncomplicated pyelonephritis have not been established. The efficacy and safety of parenteral ciprofloxacin for the treatment of uncomplicated pyelonephritis in women, were compared with those of parenteral cefotaxime. Methods In this open label, randomized trial conducted from May 2005 through August 2006, we enrolled 50 women aged at least 18 years with clinical diagnosis of acute uncomplicated pyelonephritis; 31 were included in the analysis. Patients with uncomplicated pyelonephritis were randomly assigned either to ciprofloxacin or cefotaxime. In both groups, patients could be switched to an oral agent after 7 days of parenteral study therapy. Results At 5 days posttherapy, bacteriologic cure rate were 85.7%(18 out of 21) for the ciprofloxacin group and 80%(8 of 10) for the cefotaxime-cefixime group(p>0.05). Clinical cure rate were 95.2%(20 out of 21) for the ciprofloxacin group and 100%(10 of 10) for the cefotaxime group(p>0.05). Escherichia coli, which caused more than 80% of infections, was more frequently resistant to ciprofloxacin than to cefotaxime. But it was no statistical significance(p>0.05). Drug related adverse events not occurred. Conclusions The efficacy and safety of a 14-day regimen of ciprofloxacin were equivalent to those of a 14-day regimen of cefotaxime in women patients with acute uncomplicated pyelonephritis. Ciprofloxacin and cefotaxime showed no difference in the efficiency in therapy of the acute pyelonephritis.
