하중조건과 지지골 높이에 따른 임플란트 주변골에서의 유한요소 응력분석

Abstract
다수의 임플란트 매식 후 지지골의 수직적 높이가 다른 경우 다양한 하중 조건에서 고정체와 골 계면에서의 응력 발생 및 분포에 관한 연구는 미비하였다.
본 연구에서는 구강 내 지지골의 잔존 상태에 따라 매식부위의 지지골 높이가 다른 경우 두개의 표준형 임플란트를 매식한 후 금관을 서로 연결하여 제작된 임플란트 보철물에 수직하중 및 경사 하중을 부여했을 때 지지골에 나타나는 응력을 분석하고자 하였다.
하악 제 1, 2 대구치 부위에 임플란트 고정체를 매식한 모델로서 제 2대구치 부위의 지지골 높이를 동일하게 한 모델과 동 부위 지지골 높이를 2 mm 낮게 설계한 2종의 3차원 유한 요소 모델에 대해 200 N의 수직하중 및 경사하중을 교합면의 중심, 협측 2 mm 부위에 부여한 후 지지골에 발생하는 응력을 분석하여 다음과 같은 결과를 얻었다.
1. 교합면의 중심에 수직하중이 부여되는 경우 지지골 높이와 관계없이 치밀골과 해면골에 응력이 분포되고 있으나 경사하중이 작용하는 경우에는 해면골에는 응력이 분포되지 않았다.
2. 교합면의 중심에 수직하중이 부여되는 경우 von Mises 응력은 지지골 높이와 관계없이 양측 고정체 외측 치밀골에 집중되고 있으나, 경사하중이 가해질 경우에는 양측 치밀골 설측에서 나타났다.
3. 협측 2 mm 부위에 수직하중이 부여되는 경우 고정체-해면골 계면 상부와 피질골를 중심으로 응력이 발생하나 경사 하중이 부여되는 경우 양측 고정체-지지골 계면를 중심으로 고정체의 첨단 하방 및 설측 해면골, 치밀골 상부 협측에 응력이 분포되고 양측 치밀골 외측과 설측 상부에는 von Mises 응력이 나타났다.
4. 제 2 대구치 부위 고정체에 대한 지지골의 높이 차이에 따라 교합면의 중심에 부여되는 수직하중과 협측 2 mm 부위에 부여되는 경사 하중에서 양측 고정체 사이의 치밀골 상부 응력분포 양상에는 영향을 미치지만 해면골에는 영향을 미치지 않고 있었다.
결론적으로 임플란트를 지지하는 지지골 높이 차이는 해면골의 응력분포에 영향을 미치는 요소로 작용하지 않았으나, 지지골 높이와 관계없이 하중 방향과 하중 위치는 치밀골과 해면골에 대한 응력의 분포와 집중에 영향을 미치는 요소로 나타났다.
따라서 교합면의 접촉면을 설정하는 경우 대합치의 기능교두의 방향과 매식된 고정체의 장축방향을 고려하여 하중의 집중에 의한 골 흡수 가능성을 고려하는 것이 임상적으로 유용할 것으로 사료되었다.|The purpose of this study was to analyze the distribution of stress in the surrounding bone around implant placed in the first and second molar region. Two different three-dimensional finite element model were designed according to vertical bone level around fixture (4.0mm × 11.5mm) on the second molar region. A mandibular segment containing two implant-abutments and a two-unit bridge system was molded as a cancellous core surrounded by a 2mm cortical layer.
The mesial and distal section planes of the model were not covered by cortical bone and were constrained in all directions at the nodes. Two vertical loads and oblique loads of 200 N were applied at the center of occlusal surface (load A) or at a position of 2mm apart buccally from the center (load B). Von-Mises stresses were analyzed in the supporting bone.
The results were as follows;
1. With the vertical load at the center of occlusal surface, the stress pattern on the cortical and cancellous bones around the implant on model 1 and 2 was changed, while the stress pattern on the cancellous bone with oblique load was not.
2. With the vertical load at the center of occlusal surface, the maximum von-Mises stress appeared in the outer distal side of the cortical bone on Model 1 and 2, while the maximum von-Mises stress appeared in the distal and lingual distal side of the cortical bone with oblique load.
3. With the vertical load at a position of 2 mm apart buccally from the center, there was the distribution of stress on the upper portion of the implant-bone interface and the cortical bone except for the cancellous bone, while there was a distribution of stress on the cancellous bones at the apical and lingual sides around the fixture and on the cortical bone with oblique load.
4. With the changes of the supporting bone on the second molar area, the stress pattern on the upper part of the cortical bone between two implants was changed, while the stress pattern on the cancellous bone was not.
The results of this study suggest that establishing the optimum occlusal contact considering the direction and position of the load from the standpoint of stress distribution of surrounding bone will be clinically useful.
