치석제거술 및 치근활택술에 따른 치은열구삼출액의 변화에 대한 연구

Abstract

이 연구의 목적은 치주질환 환자에서 스켈링과 치근활택술 후에 치은열구삼출액의 변화량을 평가하는 것이다. 성인치주염 환자 중 7명(평균 연령 41세)이 스켈링과 치근활택술을 받은 후 주기적으로 관리되었다. 치태지수, 치은지수, 출혈지수, 치주낭 깊이 같은 임상적 지수들이 초진시, 스켈링 1주 후, 치근활택술 2주 후, 4주 후, 6주 후에 내원시 상,하악 제 1대구치와 전치부에서 측정되었다. 치은열구삼출액은 초진시, 스켈링 1주 후, 치근활택술 2주 후, 4주 후, 6주 후에 내원시 임상적 지수를 측정하였던 부위에서 perio paper strips을 이용하여 15초간 채취되었다. 치은열구삼출량은 periotronⓡ 8000(Oraflowⓡ Inc. New-York, USA)를 이용하여 측정되었다. 측정 시기에 따른 치은열구삼출액, 치태지수, 치은지수, 출혈지수, 치주낭 깊이의 차이는 Friedman 방법을 이용하여 통계분석되었다. 치태지수, 치은지수, 출혈지수, 치주낭 깊이는 스켈링과 치근활택술 이후에 초진시보다 더 감소되었다. 치은열구삼출량은 스켈링 및 치근활택술 후에 양성적인 관계를 나타내었다(p<0.05). 측정시기에 따른 치은열구삼출액의 변화에서, 초진시와 스켈링 1주 사이에 순(협)측에서는 27.25%, 설(구개)측에서는 20.33%의 변화량을 각각 나타내었다 (p<0.05). 결론적으로, 스켈링 및 치근활택술 후에 예후를 평가하는 지침으로서 치은열구삼출액의 변화량을 측정할 수 있을 것으로 사료된다.|The aim of this study was to evaluate the change in volume in the gingival crevicular fluid (GCF) after scaling and root planing in periodontal disease patients. 7 subjects with adult periodontitis (mean age 41) finished a course of scaling and root planing, and were recalled for a follow-up. The clinical parameters such as the plaque index (PI), gingival index (GI), bleeding index (BI) and pocket depth (PD) were measured on the maxillary and mandibular anterior teeth and the first molar teeth, at the initial visit, one week after scaling, two, four, six weeks after root planing in each subject. A 15-sec sample of the GCF was collected from each test site using perio paper strips. The volume of GCF was measured using a periotronⓡ 8000 (Orafowⓡ, New-York, USA). The differences in GCF, PI, GI, BI and PD according to the measurement time was analysed using Friedman analysis. The PI, GI, BI, and PD was lower than that at the initial visit after scaling and root planing. The volume of GCF showed a positive relationship with scaling and root planing (p<0.05). According to the measurement time of the GCF, the difference between the initial visit and one week after scaling was 27.25% and 20.33% on the labial (buccal) and lingual (palatal) side, respectively (p<0.05). In conclusion, it is suggested that the volume of GCF can used as a prognostic indicator after scaling and root planing.
