진행성 위암에서의 항암화학요법의 효과

Abstract

배경 수술이 불가능한 진행성위암환자는 예후가 나쁘다고 알려져 있다. 위암치료에 있어서 최상의 지지적 치료가 평균 생존기간이 3-4개월인데 비하여 항암 치료군은 7-9개월까지라고 알려져 있다. 이 연구는 항암 치료를 받은 수술적 절제가 불가능한 진행성 위암이나 위암의 재발이 된 환자에서 치료 반응 및 생존기간의 정도를 파악하고자 하였다. 방법 2002년12월20일부터 2004년 11 월20일까지의 조선대학교 혈액종양내과에서 항암 치료를 받은 수술적 절제가 불가능한 진행성 위암이나 위암의 재발이 된 환자를 분석하여 paclitaxel ,docetaxel, capecitabine 을 사용하여 항암요법을 시행하게 되었고 정기적(항암치료 2회에서3회마다)으로 반응정도를 평가 하였다. 결과 총 환자 22명중 15명에서 2차례이상의 항암요법의 시행완료로 반응을 평가 할 수 있었으며 임상적으로 CR이 1예, PR이 6예, SD가 5예, PD가3예고 이에 총 반응은 CR 과 PR 을 합하여 7예로 반응률은 47%이었다. 전체 생존기간의 중앙값은 33.9 주(범위 24.4-43.4 주)이었으나 실제로 2회이상의 항암요법의 시행 완료로 반응을 평가 가증했던 환자의 생존기간은 39.4 주(범위 30.1-48.7 주)였다.|Background. Patient with inoperable advanced gastric cancer have a poor prognosis. Best supportive care in advanced gastric cancer results in median survival times of 3-4 months versus 7-9 months for chemotherapy. This study was performed to estimate clinical characteristics, response rate, survival duration of inoperable advanced gastric cancer or recurred gastric cancer patients who were treated with chemotherapy. Method We reviewed clinical data of twenty two patients with inoperable advanced gastric cancer and recurred gastric cancer who were treated with chemotherapy in Chosun University Hospital, hemato-oncology department between December 2002 and November 2004. They received paclitaxel, docetaxel, capecitabine and response to treatement was assessed every two or three cycle. Result Fifteen of twenty two patient were assessable for response. Clinical complete response was observed in one case and partial response was observed in six cases. Overall response rate was 47% in this study. Overall survival duration was 33.0 weeks (range: 19.5-46.5weeks) and survival duration in assessable patient was 39.4 weeks(range 30.1-48.7weeks)
