유리체절제술 후 발생한 난치성 녹내장 환자에서 아메드 밸브 삽입수술에 대한 임상분석

- Abstract
- Purpose : This study was performed to evaluate the effectiveness of Ahmed valve implantation for refractory glaucoma following pars plana vitrectomy.
Material and Method : 17 eyes of 15 patients suffering from secondary glaucoma following pars plana vitrectomy were treated with Ahmed valve implantation. All the eyes were in critical condition, and the intraocular pressure failed to control with glaucoma medication. Success of the intraocular pressure control was defined as a range of intraocular pressure 6 to 22 mmHg regardless of glaucoma medication.
Result : Mean intraocular pressure was 47.5 mmHg (25-66 mmHg) preoperatively, 13.8 mmHg (3-22 mmHg) 6 months postoperatively, and 12.9 mmHg (hypotony - 23 mm Hg) at final visit. Postoperative intraocular pressure significantly decreased(p<0.005). Preoperative numbers of glaucoma medication was 1.76. Postoperative were 0.35 at 6 months and 0.29 at final visit. Also, the number of medications decreased significantly(p<0.005). The total success rate was 83.4% at 6 months and 76.4% at final visit.
Conclusion : Ahmed valve implantation is a safe and effective method for refractory glaucoma following vitrectomy.
