보건진료원들의 요실금 관련 간호행위에 영향을 미치는 요인

Abstract

연구목적: 본 연구는 보건진료원들의 요실금 관련 지식, 태도 및 요실금 관련 간호행위 정도를 파악하고, 요실금 관련 간호행위에 영향을 미치는 요인을 조사하기 위해 실시된 서술적 상관관계 연구이다. 연구방법: 연구대상자는 광주광역시·전남에 소재한 보건진료소에서 근무하고 있는 보건진료원 330명 전수였으며, 자료수집은 구조화된 설문지를 이용한 자기기입식으로 우편설문을 통해 이루어졌다. 설문에 응한 170명(응답률: 51.5%)의 보건진료원 자료가 본 연구를 위한 분석에 사용되었다. 수집된 자료는 기술통계, t-test, ANOVA, 피어슨 상관분석과 단계적 다중회귀분석을 사용하여 분석하였다. 연구결과: 보건진료원들이 정규교육 또는 보수교육에서 요실금에 대해 받은 교육은 부족하였으며, 요실금 관련 지식설문지의 정답율은 68.96%였다. 보건진료원들의 요실금 환자를 돌보는 것에 대한 태도는 긍정적이었지만, 요실금 관련 간호행위는 소극적으로 참여하고 있었다. 기대했던 것과는 다르게 요실금 관련 지식은 요실금 관련 간호행위의 예측요인이 아니었다. 요실금 관련 태도와 요실금 관련 교육필요성 여부가 보건진료원들의 요실금 관련 간호행위의 예측요인이었으며, 이들 두 변수가 보건진료원의 요실금 관련 간호행위 변이의 9%를 설명했다. 결론: 보건진료원들은 요실금 관련 간호행위에 적극적으로 참여하고 있지 않았으며, 요실금 대상자를 간호하는 것에 대한 태도와 요실금 관련 교육요구가 이들의 요실금 관련 간호행위를 예측하는데 중요한 변수였다. 보건진료원들의 요실금 관리 능력 및 실무를 향상시키기 위한 보수교육 프로그램의 개발, 적용 및 평가가 요구된다. 보건진료원들의 요실금 관련 간호행위를 증진시키기 위한 교육 프로그램은 단순한 지식 증가에 중점을 두지 말고 태도변화를 가져올 수 있는 접근을 고려해야 한다. 요실금 관련 간호행위에 영향을 주는 요인들을 규명하기 위해 좀 더 다양한 관련 변수들을 포함한 반복연구가 필요하다.|Objective: Knowledge and attitudes related to urinary incontinence(UI) held by community health practitioners(CHPs) may influence the quality of care provided. The purposes of this study were to investigate CHPs knowledge, attitude, and practice related to UI, and to identify predictors of their practice related to UI. Methods: A descriptive-correlational study using self-administered questionnaire was conducted. For the survey, a total of 330 members of the local Korean Association of CHPs were included in this study. A mailed survey was conducted to collect data. Data were analyzed using descriptive statistics, t-test, ANOVA, correlation, and stepwise multiple regression analysis. Finding: Of the CHPs surveyed, 170(51.5%) returned completed questionnaire. Educational preparation of CHPs related to UI was limited. The average correct score of knowledge scale was 68.96%. Overall, CHPs exhibited positive attitudes toward UI, averaging 94.02 out of a possible 120(78.35%). CHPs in this study were not actively participate continence-related practice behaviors. Contrary to expectation, knowledge was not significant related to practice related to UI among CHPs. Practice related to UI among CHPs was significantly related to their attitude toward care of UI clients. In the final analysis, practice related to UI among CHPs was predicted by attitude and educational needs for UI and these two variables explained 9% of variance of practice related to UI among CHPs. Conclusion: The results of this suggest the importance of attitude and educational needs in shaping nursing practice related to UI. To facilitate practice related to UI among CHPs, efforts for attitudinal change related to UI among CHPs are needed to facilitate practice related to UI. Moreover, continuing educational program for UI management should be developed, applied and evaluated. More studies are needed to identify predictors of practice related to UI among CHPs.
