경골 간부 골절의 치료 원칙

Abstract

목적 : 경골 간부 골절은 정형외과 영역에서 가장 많은 골절중의 하나이다. 이 골절에 대해 다양한 치료 방법이 소개되어 왔으며 치료의 결과도 다양하다. 저자는 경골 간부 골절의 치료시 다양한 치료 방법을 사용하여 각각의 치료 결과와 합병증에 대해 알아보고자 하였다. 연구 대상 및 방법 : 저자는 2002년 1월부터 2003년 12월까지 조선대학교 병원 정형외과에서 추시 가능하였던 30례의 경골 간부 골절을 대상으로 하였다. 치료 방법으로는 보존적 치료로는 슬개골 부하 보조기를 사용하였고 수술적 치료로는 금속판 내고정술, 외고정기를 이용한 외고정술, 골수강내 금속정을 이용한 내고정술을 이용하였다. 결과 : 각 치료 방법에 따른 골유합시기나 재활시기등의 결과는 다양하였다. 합병증으로는 불유합 및 감염이 있었으며 환자의 만족도에 따른 치료 결과는 비교적 좋은 결과를 보였다. 결론 : 경골 간부 골절에서 치료 방법의 선택은 골절의 양상이나 동반손상의 정도에 따라 결정되어야 하며 이로서 합병증을 최소화 할 수 있고 회복 기간도 최소화 할 수 있을 것으로 사료된다.|Purpose : Tibia shaft fracture is one of the most common fracture in orthopaedic department. Many treatment options have been introduced and the results of treatment are various. This paper is to evaluate the result and complication of treatment of tibial shaft fracture with various treatment methods. Material and Methods : Author reviewed 30 cases of tibial shaft fracture at Chosun university hospital from January 2002 to December 2003. Treatment options are PTB brace, plate and screw fixation, external fixator, intramedullary nailing. Results : Each treatment option show various results in bony union time and rehabilitation time. Complications were nonunion and infection. According to patient's satisfaction, the results of each treatment options were relatively good. Conclusion : In tibial shaft fracture, treatment should be select according to fracture pattern and accompanied injury. It may be minimized the complication, and may be shorten the recovery time.
