소아에서 Lidocaine 전투여가 Rocuronium 정주시 발생하는 회피반응에 미치는 영향

- Abstract
- Background: Rocuronium produces various withdrawal responses if administered after the loss of consciousness. The aim of this study was to evaluate the effect of lidocaine pretreatment on withdrawal movements during the injection of rocuronium in pediatric patients.
Methods: Sixty ASA physical status Ⅰ and Ⅱ patients were randomly divided into two groups; group Ⅰ ( placebo; normal saline 0.1 ml/kg, n=30), and group Ⅱ ( lidocaine 0.1 ml/kg pretreatment, n=30). After general anesthesia was induced with thiopental sodium (5 mg/kg), tourniquet application on forearm was performed. Then, patients in group Ⅰ recieved normal saline 0.1 ml/kg, and patients in group Ⅱ received 1% lidocaine 0.1 ml/kg, The occlusion was released after 10 seconds, and 0.6 mg/kg of rocuronium was injected over 10 seconds. The patient's response to rocuronium injecion was graded using a 4-point scale.
Results: The incidence of withdrawal movements decreased significantly in the lidocaine pretreated group as follows: 90% in group Ⅰ and 50% in group Ⅱ (P < 0.05).
Conclusions: Lidocaine pretreatment can effectively reduce withdrawal movement on injection of rocuronium in pediatric patients.
