한말 醫藥需要를 통해 본 약값과 질병 : 전라도 강진의 朴藥局 사례

Abstract

조선후기 이래 약값은 일반 물가에 비하여 비싼 편이었다. 그래서 서민들이 약을 복용하기란 쉬운 일은 아니었다. 하지만 점차 의약소비가 대중화되어가면서, 박약국의 고객 가운데는 부유층도 있었지만, 빈한층도 적지 않았다. 의약의 원가와 판매가를 비교해 보면, 마진이 높은 편이었다. 그래서 박약국의 경우 의약을 팔아서 적지 않은 재력을 축적할 수 있었고, 그 재력으로 토지를 매입하고 사채를 운영하였다. 이러한 연유로 인해 20세기 후반까지 한국의 지방 재력가는 한약방, 그리고 양조장이나 정미소에서 나왔다. 박약국의 판매 장부에 가장 많이 등장하는 의약은 사물탕인데, 이는 부인병이나 순환기 질환 치료약이었다. 그 다음으로 많이 등장하는 것이 패독산과 쌍화탕인데, 이 두 약은 감기 치료약이다. 그리고 학질 치료약인 금계랍도 많이 팔리었다. 이로 보아 당시 兵營 사람들에게 가장 많이 노출된 질병이 순환기 질환, 감기, 부인병, 학질이었다. 한편, 병영 사람들은 박약국에서 보양강장약도 많이 사갔다. 이상의 질병실태와 건강관은 오늘날까지 별다른 차이 없이 이어져오고 있다.

The price of medicine was relatively higher than the general price level since the latter part of Joseon. It was not easy for common people to purchase medicine and take it. With the gradual popularization of medicine consumption, however, the customer list of Park's Pharmacy included members of the poor class as well as the affluent ones. When the production costs of medicine were compared with its selling prices, its margin was rather high. Park's Pharmacy was thus able to accumulate considerable wealth by selling medicine and used the wealth to purchase land and run a private loan. For this reason, local men of wealth stemmed from herbal medicine shops, brewery and rice mills in Korea until the second half of the 20th century. Samultang made the most appearance in the books of Park's Pharmacy, and it was a treatment for female disorders and circulatory system diseases. It was followed by Paedoksan and Ssanghwatang, both of which were used to treat a cold. Geumgyerap was also sold a lot to treat malaria. These findings indicate that people at the barracks were most exposed to circulatory system diseases, cold, female disorders, and malaria those days. The people at the barracks also purchased a lot of invigorating health tonic. Such conditions of illness and views of health have lasted till today with no big differences.
