고혈압 환자의 뇌졸중 및 심근경색증 조기증상 인지 관련 요인

Abstract

목표: 본 연구는 고혈압 환자를 대상으로 뇌졸중 및 심근경색증 조기증상 인지 수준을 파악하고, 관련 요인을 확인하기 위하여 수행되었다.

방법: 2019년 지역사회건강조사를 이용한 단면연구이며, 30세 이상 고혈압 환자 65,260명을 대상으로 하였다. 고혈압 환자의 사회인구학적 요인, 건강 관련 특성, 고혈압 관련 특성을 독립변수로 하고, 뇌졸중 및 심근경색증 조기증상 인지를 종속변수로 설정하였고, 복합표본분석을 이용한 카이제곱 검정과 다중 로지스틱 회귀분석을 실시하여 뇌졸중 및 심근경색증 조기증상 인지 요인을 확인하였다.

결과: 고혈압 환자의 뇌졸중과 심근경색증 조기증상 인지율은 각각 56.6%, 47.7%였다. 뇌졸중 및 심근경색증 조기증상의 낮은 인지와 공통적으로 관련이 있는 경우는 고연령, 낮은 교육수준, 배우자가 없는 경우, 기초생활수급자, 현재흡연, 좋지 않은 주관적 건강상태, 건강검진을 받지 않은 경우, 혈압수치 미인지군과 고혈압 관리교육을 받지 않은 경우 등이었다. 뇌졸중 조기증상의 인지는 30대에서 유의하게 낮았고, 심근경색증 조기증상 인지는 남자, 걷기실천을 하지 않는 경우에서 통계적으로 유의하게 낮았다.

결론: 고혈압 환자의 뇌졸중 및 심근경색증 조기증상 인지는 연령, 교육수준, 배우자 여부, 건강행태와 고혈압에 대한 관리가 유의한 관련이 있었다. 고혈압 조절을 위한 체계적인 관리와 함께, 관련 특성을 고려한 맞춤 교육이 진행된다면 심뇌혈관질환에 대한 인지도 향상과 더불어 발생 시 적절한 조치를 취할 수 있도록 하는 데 도움이 될 수 있을 것으로 생각된다. |Objective: This study was performed to determine the levels of awareness of early symptoms of stroke and myocardial infarction and to identify their related factors in hypertensive patients.

Methods: A cross-sectional study design was applied using the 2019 Community Health Survey, and the study subjects were 65,260 hypertensive patients aged 30 years or older. Socio-demographic factors, health-related characteristics, and hypertension-related characteristics of hypertensive patients were determined in independent variables, and the dependent variables were awareness of early symptoms of stroke and myocardial infarction. The chi-square test and multiple logistic regression analysis using complex sample design were performed to identify factors associated with awareness of early symptoms of stroke and myocardial infarction.

Results: Awareness rates of early symptoms of stroke and myocardial infarction in hypertensive patients were 56.6% and 47.7%, respectively. The commonly associated factors with lower awareness of early stroke and myocardial infarction symptoms were older age, lower educational attainment, not living with a spouse, being a basic living security recipient, current smoking, poor self-rated health, not having a regular health checkup, unrecognized blood pressure level and not being educated for the management of hypertension. Awareness of early stroke symptoms was statistically significantly lower in the 30s, and awareness of early myocardial infarction symptoms was statistically significantly lower in men and those who did not practice walking activity.

Conclusion: Awareness of early symptoms of stroke and myocardial infarction in hypertensive patients was significantly associated with age, educational attainment, living with spouse, health behaviors, and management of hypertension. It will be necessary that systematic management of hypertension and patient-specific health education to increase awareness of cardiovascular disease and take appropriate action when it occurs.
