코로나19 유행 시기에 당뇨병 환자의 합병증 검사 수검 관련 요인

Abstract

당뇨병은 주요 만성질환 중 하나로 지속적인 합병증 관리가 중요하다. 코로나19 유행과 장기화는 당뇨병 환자의 합병증 관리에 대한 우려를 불러일으켰다. 이에 본 연구는 국내 당뇨병 환자를 대상으로 코로나19 유행 시 당뇨병 합병증 검사 수검 및 관련 요인을 조사하였다. 2021년 지역사회건강조사 자료를 사용하였으며, 2021년 참여자 229,242명 중 당뇨병 진단을 받은 과거력이 있는 28,477명을 분석하였다. 당뇨병 합병증 검사는 안저검사와 미세단백뇨 검사가 포함되었다. 일반 특성과 코로나19 관련 특성에 따른 당뇨병 합병증 검사 수검률을 비교하기 위해 카이제곱 검정을 실시하였다. 그리고 다중로지스틱회귀분석을 통해 검사 관련 요인을 파악하였다. 전체 대상자의 안저검사 수검률은 42.7%, 미세단백뇨 검사 수검률은 51.7%였다. 코로나19로 인한 신체활동 증가(OR=1.16, 95%CI=1.02-1.32), 인스턴트 식품 섭취 감소(OR=1.20, 95%CI=1.03-1.41), 음주 빈도 감소(OR=1.24, 95%CI=1.01-1.53), 코로나19 감염으로 인한 염려(OR=1.11, 95%CI=1.02-1.20), 코로나19 백신 접종을 한 경우(OR=1.20, 95%CI=1.07-1.34) 안저검사를 받을 가능성이 높았다. 그리고 코로나19로 인해 인스턴트 식품 소비가 감소한 경우(OR=1.16, 95%CI=1.00-1.36), 코로나19 감염으로 인한 비판을 우려하는 경우(OR=1.10, 95%CI=1.00-1.20) 미세단백뇨 검사를 받을 가능성이 높았다. 본 연구는 코로나19 유행 기간 동안 당뇨병 환자의 당뇨병 합병증 검사 현황 및 관련 요인을 평가하였다. 합병증 검사 수검은 일반 및 당뇨병 관련 특성뿐 아니라 코로나19 관련 특성과도 관련이 있었다. 본 연구 결과는 신종 감염병 시대에 만성질환 합병증 관리의 근거자료로 활용될 수 있을 것이다.|Objective: Diabetes is one of the burdensome chronic diseases, and continuous management of complications is important. The COVID-19 pandemic and its prolongation have raised concerns about the management of complications in patients with diabetes. This study investigated the examination of diabetes complications and related factors during the COVID-19 epidemic in Korean diabetic patients. Methods: We used data from the 2021 Korea Community Health Survey. Of the 229,242 participants in the 2021 survey, 28,477 people with a diagnosis of diabetes were analyzed. Examination of diabetes complications included eye fundus examination and microproteinuria test. A chi-square test was conducted to compare the proportion of examinations of diabetes complications according to the general and COVID-19-related characteristics. And examination-related factors were identified through multiple logistic regression analysis. Results: Overall, 42.7% underwent fundus examination, and 51.7% underwent microalbuminuria examination. People with increased physical activity due to COVID-19 (OR=1.16, 95%CI=1.02-1.32), decreased consumption of instant food (OR=1.20, 95%CI=1.03-1.41), decreased drinking frequency (OR=1.24, 95%CI=1.01-1.53), those who were concerned about COVID-19 infection (OR=1.11, 95%CI=1.02-1.20), and those who have received COVID-19 vaccination (OR=1.20, 95% CI=1.07-1.34) were more likely to undergo fundus examination. And those with decreased consumption of instant food decreased due to COVID-19 (OR=1.16, 95%CI=1.00-1.36), and those who were concerned about criticism due to infection with COVID-19 (OR=1.10, 95%CI=1.00-1.20) were more likely to undergo microalbuminuria test. Conclusion: This study evaluated the examination status of diabetes complications and related factors in diabetic patients during the COVID-19 epidemic. Examination uptake were related to COVID-19-related characteristics as well as general and diabetes-related characteristics. This study results could be used as evidence for complication management for chronic diseases during the era of new infectious diseases.
