정신과 폐쇄병동 간호사의 자기 돌봄 경험에 대한 현상학적 연구

Abstract

The purpose of this study is to explore the essential structure of experiences in which nurses in the closed psychiatric ward realize self-care and practice self-care in nursing behavior. The research question is, 'How is the self-care experience of nurses in the closed psychiatric ward?'. In this study, the following were selected as research participants: those who are currently working in national mental hospitals, psychiatric hospitals, and mental hospitals located in cities S, Y, G, and C, those who have more than 3 years of experience in psychiatry, those who have felt the need for self-care through prior questions and have experience in self-care, and those who understand the purpose of the research, voluntarily agree to the research and can share their self-care experiences.The number of interviews was from 1 to 3 times, and the initial interview time ranged from 50 minutes to 1 hour and 30 minutes. The 2-3rd interview time ranged from 40 minutes to 1 hour and 10 minutes. This study derives meaningful statements from the original data collected using Colaizzi's method of data analysis. There were a total of 53 subjects, followed by 18 subject groups and 5 categories. The five categories were identified as: ‘Try to find the identity as a nurse who plays a therapeutic role in the closed ward’, ‘Become a nurse who cannot play a therapeutic role and is confused about one's identity’, ‘Reflect oneself who loses power and hope as a psychiatric nurse’, ‘Realize care for oneself and focus on oneself in search of inner vitality’, ‘Try to fill the depleted power of the closed psychiatric ward in one's way’. The self-care of the nurses in the closed psychiatric ward draws one's inner power through self-respect, and through this, care for patients is achieved eventually. In other words, this is a process in which the nurses and the patients interact with each other. The self-care of the nurses in the closed psychiatric ward first awakens the vitality among themselves, thereby restoring the therapeutic relationship as a nurse. And then, the therapeutic impact of restoring the relationship with a patient in a closed ward creates the hope of living for the patient to return to the community.|본 연구는 정신과 폐쇄병동 간호사들이 자기 돌봄을 자각하고, 간호행위 속에서 자기 돌봄을 실천해 나가는 경험의 본질적 구조를 탐색하기 위해 시도 되었다. 연구문제는 '정신과 폐쇄병동 간호사의 자기 돌봄 경험은 어떠한가?'이다. 본 연구에서는 연구 참여자를 S시, Y시, G시, C도에 소재한 국립정신병원 및 정신전문병원, 정신과 병원에서 현재 근무 중인 자, 만 3년 이상의 정신과 경력을 가진 자, 사전 질문을 통해 자기 돌봄의 요구를 느끼고, 자기 돌봄을 수행한 경험이 있는 자, 연구의 목적을 이해하고 연구에 자발적으로 동의하며 자신의 자기 돌봄 경험을 이야기할 수 있는 자로 선정하였다. 면담횟수는 1회에서 3회였고, 최초 면담시간은 50분에서 1시간 30분 정도까지 이루어졌으며, 2-3회 면담시간은 40분에서 1시간 10분이었다. 본 연구는 수집된 원자료로부터 Colaizzi의 분석방법으로 의미 있는 진술을 도출하였다. 이에 따른 주제는 총 53개였고 18개의 주제군과 5개의 범주로 나타났다. 5개의 범주는 ‘폐쇄병동에서 치료적 역할을 하는 간호사의 정체성을 찾기 위해 노력함’, ‘치료적 역할을 할 수 없는 간호사가 되어 자기 정체성(正體性)에 대해 방황함’, ‘정신과 간호사로서 동력과 희망을 잃어가는 자신을 돌아봄’, ‘자신(self)를 위한 돌봄을 깨닫고 내면의 생동성(生動性)을 찾아 자신에게 집중함’, ‘정신과 폐쇄병동의 고갈된 동력을 자신의 방법으로 채우려함’으로 확인되었다 본 연구 결과 정신과 폐쇄병동 간호사의 자기 돌봄은 결국 자기 내면의 동력을 이끌어내는 자기 존중을 통해서 비로소 환자를 위한 돌봄이 이루어지는 것이며 이것은 곧 환자와의 상호영향을 주고받는 과정임을 알 수 있었다. 이러한 정신과 폐쇄병동 간호사의 자기 돌봄은 우선 자신 속에서 스스로 생동성을 일깨워서 간호사로서의 치료적 관계를 회복하게 만든다. 그리고 폐쇄병동에서의 환자와의 관계 회복을 통한 치료적 영향력은 환자로 하여금 지역사회로 복귀하여 살아갈 수 있는 삶의 희망을 만들어 준다.
