Abstract

Introduction: The periapical and fascial space abscess are one of the infectious diseases that occur in the oral maxillofacial area. The object of this study is to analyse the bacteria of periapical bascess and fascial space abscess using multiplex real-time polymerase chain reaction (MRT-PCR) before and after drainage and antibiotic therapy.

Method: I studied 30 patients who were diagnosed as periapical abscess or fascial space abscess. All patients were treated with antibiotics following incision and drainage. Bacterial culture and MRT-PCR analysis were performed after collecting pus specimen. Two days after the drainage, abscess was reapplied and MRT-PCR analysis was performed.

Result: The results of MRT-PCR analysis and bacterial culture were consistent in 25 of 26 patients(96.2%) except for 4 patients with failed culture. In all 30 patients who underwent drainage, MRT-PCR showed that the dominant bacterium 1 and 2 showed a decrease in the number of specimens 2 days after the drainage, but the survival rate of the bacteria ranged from 1.81% to 67.52%.

Conclusion: This result demonstrated that MRT-PCR may play a role as an indicator of progress in patients with odontogenic infection. But target bacteria that can be detected by MRT-PCR are limited, so it is not suitable to use MRT-PCR alone for treating odontogenic abscess. However, MRT-PCR may play an auxiliary role in predicting patient progress and determining appropriate antibiotics.
