국내 종합병원 간호사에서 의료기관인증제의 경험이 한자안전문화에 미치는 영향

Abstract

Purpose : The purpose of this study was to investigate the impact of the experience of hospital accreditation at general hospitals on nurses' levels of perception of each dimension of patient safety culture and the relationship of hospital accreditation with the overall perception of safety and the hospital patient safety grade as a result related to patient safety culture, and identify the factors which affect patient safety culture.

Methods : This study was carried out by conducting a survey among a total of 320 nurses working in the six hospitals located in G Metropolitan City, and measuring the dimensions of patient safety culture using the questionnaire of the Hospital Survey on Patient Safety Culture, which consisted of 44 items. After the approval of the IRB, data were collected from July 28 to September 27 in 2014, and they were analyzed using SPSSIBM 20.0 program.

Results : In this study, the mean score for general hospital nurses' overall perception of safety was found to be 3.53 points, and when the levels of perception for the dimensions of patient safety culture were assessed by average percentages of positive responses, the frequency of events reported(90.5%), the immediate supervisor/manager(69.4%), feedback and communication about errors(66.3%), staff(61.6%), overall perception of safety(55.7%), teamwork across hospital units(51.8%), teamwork in the hospital (50.7%), hospital management support for patient safety(43.7%), hospital hand-offs and transitions(43.6%), communication (43.2%), organizational learning and continuous improvement (35.6%), hospital patient safety grade(34.6%) and the non-punitive response to error (23.0%) were reported in the order listed. For the dimensions of patient safety culture, which were added in Model 2 concerning the dependent variable 'overall perception of patient safety' in the hierarchical multiple linear analysis in order to examine the impact of the experience of hospital accreditation on patient safety culture, the working environment (β=0.589, p=<.001) and hospital management support for patient safety (β=0.261, p=<.001) were observed to increase the explanatory power of the model statistically significantly (ΔR2=0.423, F(p)=26.59(<.001), and hospital accreditation was found to be unrelated to the hospital safety grade with the adjusted R2 value of 0.007 although it was selected as the predictor which had a slight effect on 'overall perception of safety' (β=0.097).

Conclusion: In this study, general hospital nurses' experience of hospital accreditation was found to have insignificant effects on overall perception of patient safety and was found to be unrelated to the hospital patient safety grade. In order to evaluate the effects of the policy intervention of hospital accreditation, it is necessary to conduct a longitudinal study or a time series study to investigate changes in nurses' perception of patient safety culture and overall perception of patient safety and identify the factors which affect the hospital patient safety grade.|Purpose : 본 연구는 종합병원에서 의료기간인증제 경험 여부가 간호사들의 환자안전문화 영역별 인식 수준과 환자안전문화결과로써 전반적인 안전인식과 병원의 환자안전등급과의 관련성을 조사하고, 환자안전문화에 미치는 영향 요인을 파악하고자 한다. Methods : 본 연구는 G광역시에 소재한 종합병원의 의료기관 6곳에서 320명의 간호사를 대상으로 44문항의 Hospital survey on patient Safety Culture(AHRQ.2004)설문지를 이용하여 환자안전문화 영역을 측정하였다. IRB 승인 후 2014년 7월 28 - 9월 27일 동안 자료 수집하였고, SPSSIBM 20.0 프로그램을 이용하여 분석하였다. Results : 본 연구에서 종합병원 간호사들의 ‘안전의 전반적인 인식’ 평균 점수는 3.53점이며 환자안전문화 영역별 인식 수준을 긍정 응답률로 확인한 결과, 사건보고의 빈도(90.5%), 직속상관_관리자(69.4%), 피드백 및 오류에 대한 통신(66.3%), 직원(61.6%), 안전의 전반적인 인식(55.7%), 병동에서 팀웍(51.8%), 병원에서 팀웍(50.7%), 환자안전에 대한 병원 경영 지원(43.7%), 교대근무 및 인수인계(43.6%), 의사소통(43.2%), 조직학습_지속적인 개선(35.6%), 병원의 환자 안전 등급(34.6%), 오류에 대한 비 징벌(23%)순으로 보고되었다. 의료기관 인증제경험이 환자안전문화에 미치는 영향을 확인하기 위해 위계적 다중 선형분석에서 종속변수 ‘전반적인 환자안전 인식’에서 모델2에서 추가된 환자안전문화영역은 근무환경(β=0.589, p=<.001)과 환자안전에 대한 병원 경영 및 지지(β=0.261, p=<.001)은 모델의 설명력을 통계적으로 유의하게 증가시켰고(∆R2=0.423, F(p)=26.59(<.001) 환자인증제 여부는 Adjusted R2 변화량이 0.007(p<.001)으로 ‘안전에 대한 전반적인 인식’에 미미한 영향을 미치는 예측요인(β=0.097)으로 선정되었으며 인증제 여부는 병원의 안전등급과 관련이 없었다. Conclusion: 본 연구에서 종합병원 간호사를 대상으로 한 의료기관인증제 경험은 ‘전반적인 환자안전 인식’에 미미한 수준의 영향을 미치고, ‘병원의 환자안전 등급’에는 관련이 없는 것으로 조사되었다. 이러한 결과는 의료기관 인증평가를 수행하기에는 인적 자원뿐만 아니라 시스템적 인프라구조가 형성되지 않은 상태로 보건당국은 의료의 질 향상을 위한 촉진 요인이 필요하고 의료기관인증경험이 전반적인 환자안전 인식 및 병원의 환자안전등급에 미치는 영향에 대한 추후 연구가 필요하다.
