- A study of clinical efficacy of
parenteral nutrition with lipid containing fish oil
in newborns

Objectives: To reduce clinical complications of parenteral nutrition(PN) by choosing the best lipid emulsion.
Methods: We retrospectively analyzed 66 newborns received PN with either fish oil group(SMOFlipidⓇ) or olive oil group(ClinOleicⓇ) for at least 7 days in NICU at Chosun University Hospital from January 2011 to July 2014, except for those who received lipid emulsion from 5 days after birth. Clinical complications and serum biochemistry after PN were retrospectively investigated by medical records.
Results: 66 patients were included in the study. The study group was supplied fish oil-containing lipid emulsion and the control group was not. In control group vs study group, the number of prematurity was 38(90.5%) vs 20(83.3%), and an average of birth weight was 1.60 kg vs 1.68 kg.
Diret bilirubin(DB) was uncomparable, because of the minority. Total bilirubin(TB) was decreased after PN in ClinOleicⓇ group, but increased in SMOFlipidⓇ group. ALP and TG was increased and GGT was decreased in both groups.
The rate of cholestasis in ClinOleicⓇ group was higher(9.5%) than study group(4.2%), but ROP, sepsis, and NEC were more frequent in control group. And there was no statistically significance.
Conclusion: In this study, there is no statistically meaningful data in the effect of each lipid emulsions. But considering the efficacy of ω-3 fatty acid, we need to try another lipid emulsion which contains more higher content of fish oil. Then a further study with a larger group is needed which might be conducted prospectively, and we should check necessary laboratory findings regularly which are needed for the diagnosis of complications after long period of PN.
