전방 재설정 장치의 종류에 따른 저작근의 근전도 비교

Abstract

중심위는 보철학 영역에서 중요한 의미를 가지는 하악위(mandibular position)로써 전방 재설정 장치(anterior deprogramming device)를 사용하여 환자 스스로 교합력을 가할 시 과두가 관절와 내의 최전상방위에 위치하기 때문에 중심위를 쉽게 채득할 수 있다. 때문에 이러한 전방 재설정 장치는 임상적으로 자주 사용되고 있으며, 이러한 장치들로는 최근 Lucia jig, leaf gauge, cotton roll이 대표적으로 이용되고 있다. 본 논문의 목적은 중심위 유도 시 사용되는 전방 재설정 장치 중 Lucia jig, leaf gauge. cotton roll을 이용하여, 표층 교근과 전방 측두근의 근활성도를 근전도 기계(Electromyography)로 측정하고, 장치의 유효성을 비교 및 평가해 보는 것이다. 심한 부정교합 및 턱관절 장애가 없고, 제 3대구치를 제외한 상실치가 없는 건강한 성인 20명(남:여=14:6)을 피실험자로 선정하였다. 좌, 우 표층 교근 및 전방 측두근 부위에 electrode를 부착시킨 후, 연하를 통해 하악을 중심위로 유도시켜 근활성을 측정하였다. 3가지 전방 재설정 장치를 각각 장착 한 후, 연하력을 통해 환자 스스로 중심위로 유도하고, EMG(Electromyography)를 이용하여 근활성도를 측정하였다. 결과는 다음과 같다. 1. 대조군인 연하 시에 비하여, Lucia jig 및 leaf gauge 사용 시, 측두근 및 교근 모두에서 유의할 근 활성 감소를 나타내고, cotton roll 사용 시에는, 우측 측두근과 교근에서만 유의한 근활성 감소가 나타났다.(P<0.05) 2. 각 장치간 비교 시, Lucia jig, leaf gauge, cotton roll 순으로 근활성이 증가하였으며, 특히 Lucia jig와 cotton roll 비교 시, 좌,우 측두근과 좌측 교근에서 유의한 차이가 나타났다.(P<0.05) 3. 대조군인 연하시에 비해서, Lucia jig 및 leaf gauge 사용 시, 좌우 측두근 및 교근 모두에서 근균형성(muscle balance)이 유의적으로 증가하였다.(p<0.05) 4. 각 장치간 비교 시, Lucia jig, leaf gauge, cotton roll 순으로 좌,우 근 균형성이 감소하였으며, 특히 Lucia jig 및 leaf gauge 사용 시, 측두근과 교근 모두에서 cotton roll에 비해서 유의적으로 높은 근균형성이 관찰되었다.(p<0.05)|A centric relation can be described as a mandibular position playing an important role in prosthodontics, and it is spontaneously acquired while biting an anterior deprogramming device that makes a condyle locate in the most anterior upper part of articular fossa. Therefore, an anterior deprogramming devices such as Lucia jig, leaf gauge and cotton roll are widely used in clinics. The purpose of this study is to use Lucia jig, leaf gauge, and cotton roll among the anterior deprogramming devices being used to induce the centric position in order to measure the activity of superficial masseter muscle and anterior temporalis muscle with electromyography(EMG) and evaluate their effectiveness comparatively. Twenty (male:female=14:6) healthy adults having no severe malocclusion, temporomandibular disorder, or missing tooth except for the third molar were selected as test subject group. As a control group, muscle activity was measured with EMG at a centic position of mandible guided by swallowing after sticking electrodes to left and right superficial masseter muscle and anterior temporalis muscle. Then, muscle activity using 3 types of anterior deprogramming devices were measured, respectively.

The conclusions are listed as follows.

1. Comparing to the control group, muscle activity with Lucia jig and leaf gauge was significantly decreased in both temporalis and masseter muscle. But, muscle activity with cotton roll was significantly decreased in right side temporalis and masseter muscle(P<.05).

2. In comparison of each device, muscle activity increased in order of Lucia jig, leaf gauge, and cotton roll. Especially, significant difference appeared between left and right side temporalis muscle and left side masseter muscle(P<.05).

3. Comparing to the control group, muscle balance with Lucia jig and leaf gauge was significantly increased in both left and right side temporalis and masseter muscle(P<.05).

4. In comparison of each device, left and right side muscle balance decreased in order of Lucia jig, leaf gauge, and cotton roll. Especially, muscle balance using Lucia jig and leaf gauge was significantly increased in both side temporalis and left side masseter muscle than that of cotton roll(P<.05).
